FASA team insurance is provided through Chappell Insurance Agency. Prices are per team and depend on age group and deductible.

Good to know
- SAM (Sexual Abuse and Molestation) coverage is an added fee of $10.00. League SAM coverage is an added fee of $8.00 per team.
- To qualify for the league discount rate, 6 or more teams must purchase insurance at the same time.
- 100% of teams must be insured. The certificate will be issued in the League Name.
- Payment must be made with one check.
- All rates include an administration fee and registration fee.
- There is a $5.00 additional processing fee for credit card purchases.
